The Patient Coordinator (PC) is a trained Community Paramedic (CP) who is provided with extra training to work within the Mobile Integrated Healthcare- Coordination Center ( The Patient Coordinator (PC) will be responsible for a myriad of tasks and duties directly related to the Mobile Integrated Healthcare- Coordination Center ( The Patient Coordinator (PC) will work under limited guidance and supervision and will provide guidance and support to Community Paramedics (CP), crews, and regulated community healthcare providers. The goal of is to ensure that patients receive the right care, at the right time, by the right provider and are linked with appropriate resources to avoid reliance on the acute care system.
