About the role

This role directly supports national priorities to strengthen hospital‑to‑home pathways and shift care closer to home by expanding community‑based care while reducing pressure on acute services.

As an Advanced Clinical Practitioner, you will provide autonomous clinical coverage across the inpatient pathway – Windsor Intermediate Care Unit (WICU) and Seacole Community Hospital – and support Virtual Ward and Urgent Community Response. There will be a strong focus on admission avoidance.
