Licensed Practical Nurse (LPN)

Purpose or Summary Statement of Primary Duties and Responsibilities of Position

The Licensed Practical Nurse (LPN) provides direct nursing services to medically fragile students and other duties as assigned under the supervision of a licensed registered nurse. Duties are to be performed in accordance with the standards of professional school nurse practice, district/state, board of education policies and procedures and the State of Illinois Nurse Practice Act.

Principal Accountabilities

• Provides direct health services to students according to their IEP/504 plans.
• Monitors and provides appropriate care to medically fragile students to and from school and throughout the school day.
• Maintains a safe and healthy environment for all students.
• Maintains health records in accordance with the Chicago Public Schools records policy.
